You have opted for online treatment by one of our therapists. It is a legal requirement for a healthcare provider to enter into a treatment agreement with its client. To meet this obligation, and to ensure that the speech therapy (including examination) runs smoothly, we would like to make a number of arrangements with you. Would you like telephone support in completing this treatment agreement? If so, please contact your own TinyEYE contact person.
